Membership discount is valid on all items available on the sales floor and Centerville Coin & Jewelry Connection’s website, www.ccjc.com. Numismatic items, bullion and all clearance sale items are excluded from the membership discount. Membership discount may not be combined with any other specials and/or offers including, but not limited to, any sale items, special combination purchase discounts, promotions, fundraising nights, private parties, Black Book certificates, gift cards or coupons. Membership discount is not valid on any services including jewelry repair, watch repair, battery replacements, engraving and any products and/or materials used in conjunction with such services. Sales tax and shipping is always excluded from any form of discount.

Membership return policy is 30 days same as cash, excluding shipping and insurance fees. Receipt must be presented to receive refund. To be eligible for the 30 day return option, all items must be in manufacturer condition and show no signs of wear. Final decision on returns will be made by a Centerville Coin & Jewelry Employee. Standard shipping to anywhere in the continental United States is free for Charter Membership holders. Any upgrade to shipping will be charged based off of Centerville Coin & and Jewelry’s normal shipping policy. International locations, Alaska and Hawaii are not included in the Charter Member free shipping policy.

Members will not be eligible for both a Charter Membership Card and a Rewards Card. Members upgrading from a Rewards Card to a Charter Membership Card will have 30 days from the time the Charter Card has been activated to redeem all points on their Rewards Card. Rewards points cannot be used in conjunction with the Charter Membership Card discount. Reward points are not eligible to be used to purchase a Charter Membership.

Only members of the household listed on membership form and/or card can use membership discount. Temporary or current membership card provided by either Centerville Coin & Jewelry Connection or Swarovski must be shown at time of purchase in order to receive the discount at local retail store. Membership ID number may be required at time of purchase to receive discount online. Valid form of I.D. with address must also be shown at time of purchase at local retail store. Address on I.D. must match address on Membership Card. To receive Membership discount online, order must be placed using valid activated Membership account. You are responsible to report any change in information to Centerville Coin & Jewelry. Temporary membership card is valid for up to six (6) weeks. If a permanent card is not received from Swarovski by six (6) weeks, you must contact Centerville Coin & Jewelry Connection immediately. The twenty percent (20%) discount is only valid until Dec. 31, 2012 at end of business. Only Charter Memberships purchased during open enrollment will qualify for this special store discount card. If you lose your membership card you are responsible for contacting Centerville Coin & Jewelry Connection for a replacement. A replacement fee of $10.00 may be charged at such time. These terms are non-negotiable.

Any questions regarding this agreement and/or the terms and conditions listed within must be addressed before signing this form. The membership is non-returnable, non-refundable and non-transferable, and cannot be redeemed for cash or credit. All sales of memberships are final.

Any misuse of the membership card and/or the privileges associated with it, whether improper or illegal, will result in termination of the membership by Centerville Coin & Jewelry Connection with no refund of any Membership cost.

By purchasing the Charter Membership, you unconditionally agree that decisions of Centerville Coin & Jewelry Connection and their employees are final on all matters of interpretation, fact, and procedure in respect to the Charter Membership. Centerville Coin & Jewelry Connection staff will provide you with a copy of this agreement upon request. A form listing the benefits and commonly asked questions of the Charter Membership will be made available at the following locations: on Centerville Coin and Jewelry Connection’s Facebook page under the user name Coins4me or by going to www.facebook.com/coins4me, on the Centerville Coin & Jewelry website www.ccjc.com, as well as at our retail location.
